Continuity of Care Project Manager

Location: South Australia

Employment type: Part time (30 hours per week). Work From Home arrangements available; Fixed term until 30th June 2025

Salary: $79,575.74 per annum, plus superannuation and excellent salary sacrifice opportunities

SAWCAN is seeking an experienced and strategic Continuity of Care Project Manager to lead a transformative project that enhances continuity of care across the region. This is a unique opportunity to work collaboratively with Aboriginal Community Controlled Health Organisations (ACCHOs), community members, and government entities to drive meaningful change.

About the role

As the Continuity of Care Project Manager, you will lead and coordinate testing activities aimed at improving continuity of care protocols for Aboriginal families. This critical role serves as a bridge between communities, ACCHOs, and health services, ensuring that the experiences of Aboriginal mothers and children, particularly from regional and remote areas, are at the centre of the project’s design and implementation.

Key responsibilities

  • Co-design and implementation – Support the co-design of the Continuity of Care Protocols Program, ensuring that the voice of the ACCHO sector is central to the strategy.
  • Collaboration – Work closely within the SAWCAN Partner Organisations, testing networks, and SA Health to ensure a collaborative approach to project implementation.
  • Active participation – Engage in testing activities, sharing tools, resources, and best practices to enhance program outcomes.

What we’re looking for

  • Demonstrated experience in the ACCHO space, particularly in maternal and child health.
  • Strong project management skills, with a focus on system improvement and collaboration.
  • Experience in co-designing health programs with Aboriginal communities and key stakeholders.
  • An understanding of systems thinking, governance, and partnership management.
  • Familiarity with data-sharing protocols and health information systems, such as My Health Record (MHR).
